You can start with as many as 48 special dates and occasions with this form. | |

Your Membership Card has your ID# and you will transfer this to the Occasions Form to identify and validate that you are a fully paid member for life. | |

Mail in the form to start receiving your reminder cards for life. | |

All the information that you have placed on the form will go into the computer to activate your membership. | |

That's all there is to do with the Occasions Form. After you fill it out and send it in you will start receiving your reminder cards. |
